As the appointed Insurance Brokers to the Council of Clinical Hypnotherapists, “Made Easy Financial Group” are delighted to confirm that ‘Insurance Made Easy’ has negotiated a combined Medical Malpractice and Public & Products Liability package underwritten by Lloyds in London, via ProRisk (Professional Risk Underwriting Pty Ltd) in Melbourne.

This facility allows practitioners to choose varying levels of coverage starting from $1m through to $10m and it should be noted that the Limit of Liability selected applies to all risks under the policy. Therefore, if you have previously held $1m of Malpractice coverage and $10m Public Liability, you can now purchase one level of coverage that will apply across the policy.

1. Download & read the: POLICY DOCUMENT – to ensure that you understand the coverage and find it suitable to your needs. Should you have any questions relating to the policy please do not hesitate in contacting our office.
2. Download & read the: MODALITIES DOCUMENT – to ensure that you understand the coverage and find it suitable to your needs. Should you have any questions relating to the policy please do not hesitate in contacting our office.
3. Download & read the: INSURANCE RATES – applicable for the current period and establish the cost of your required coverage.
4. Download, fill in and sign the: PROPOSAL FORM
5. Download, fill in and sign the: CLAIMS DECLARATION FORM
6. RETURN  your completed PROPOSAL & DECLARATION forms to the postal address, or by facsimilie, or by scanning and attach to email: Insurance Made Easy will  advise you of acceptance of your Proposal and arrange for issuance of your Policy.
